Report a safeguarding concern

Who this is for: Guests and members

Anyone may use this form, signed in or not, to tell us about anything that worries them about the safety or wellbeing of a child or an adult at risk at a DJJS program, centre or online space. You do not have to be sure.

Before you start

  • If someone is in danger right now, call the emergency services first. The page shows the number for your region at the top (911 on the United States site, 999 on the United Kingdom site). Then tell us here.
  • Only What happened? is needed. Everything else is optional.
  • Open the form from Report a safeguarding concern in the footer of any page.

Steps

  1. In What happened? (in your own words), describe what worries you.
  2. Tick Someone may still be at risk if that is so.
  3. If you can, fill in When did it happen?, Where? and Who was involved? There is no need to name a child.
  4. If you wish, add Your name and How can we reach you? (an email or a phone number).
  5. Choose Send the report.

What happens next

  • You see "Thank you. Your report has reached us." and a reference that begins with SG. Write it down. It is shown once, on that page, and is not emailed to you.
  • Your region's safeguarding lead and every Super Admin are told at once by email. The email carries only the reference, and says whether you ticked that someone may still be at risk. It never carries what you wrote. The page names your region's safeguarding lead when one has been set.
  • What you write is kept encrypted. It is read only on the safeguarding desk, by your region's safeguarding lead and the Super Admins, and each time that desk is opened the visit is recorded.
  • A report goes through four stages on the desk: New, Being looked into, Referred to the authorities, and Closed. You are not told the stage afterwards.
  • If you left a way to reach you, the safeguarding lead may contact you.
  • If you are signed in, the report is linked to your account whatever you type, and the desk can see that it came from a signed-in member. If you leave Your name and How can we reach you? empty, the name and email of your account are kept with it too. To report with nothing linking it to your account, sign out first.
  • If you are not signed in, nothing identifies you unless you write it.
  • Signed in or not, a coded form of your connection address is kept to limit repeated reports. It is never shown on the desk.
  • A report cannot be edited once sent. To add something, send another report.

If something looks different

  • The page says several reports have come from here this hour. Reports from one connection are paused for a short while. If someone is in danger, call the emergency services. Otherwise try again later.
  • The page says the form is being set up. The service is not ready on the site just now. If someone is in danger, call the emergency services.
  • The page asks you to describe what happened. The first box must hold a few words before the report can be sent. Your other answers stay on the page.
